What is Support Coordination?
Support Coordination is a Capacity Building Support. It offers NDIS participants the opportunity to make the best use of their support in the NDIS plan.
Support Coordination helps you to:
- Understand and use your NDIS plan to pursue your goals
- Connect you with NDIS providers, community, mainstream and other government services
- Build your confidence and skills to use and coordinate your supports and live autonomously.
- Get assistance that is matched to your goals and requirements.
There are three types of Support Coordination:
- Support Connection
- Coordination of Supports
- Specialist Support Coordination.
Who is eligible for Support Coordination?
The following participants are most likely to receive Support Coordination in their NDIS plan:
- Who are Aboriginal or Torres Strait Islander heritage
- Who are with extremely complex needs and receiving high support.
- Who are with episodic mental or ill-health support needs
- Who requires regular crisis management.
- With current or past child protection or criminal justice involvement
- With a history of changing and challenging support provision
- With psychosocial disability.
